Boscobel, WI – Brady Ray Jacobs, age 23 days died on Wed. May 14, 2014 at Mayo Clinic Hospital in Rochester, MN due to complications from a hypo plastic left ventricle. Brady was born on April 21, 2014 in Rochester, MN the son of Dwayne and Elizabeth (Moore) Jacobs of Boscobel.
Brady is survived by his parents, brother Sam at home, step-sister Angel Jacobs of Boscobel, maternal grandparents Chris and Rita Moore of Fennimore, paternal grandmother Jane Jacobs of Prairie du Chien, uncles; Christopher Moore of Fennimore, Charles Moore of Fennimore, Scott (Evelyn) Jacobs of Prairie du Chien, Leroy Jacobs of Boscobel, and Willie Jacobs of Sextonville, WI, as well as many special nieces and nephews.
Brady was preceded in death by his grandpa George Jacobs.
Funeral services will be held at 12:00 noon on Monday May 19th, 2014 at the Larson Family Funeral Home in Fennimore with burial to follow in the Prairie Cemetery. Brady’s uncle, Pastor Scott Jacobs with the Apostolic Lighthouse Church of Prairie du Chien will be officiating. A visitation will be held from 10:00 A.M. until the time of service at the funeral home on Monday morning.
